• Who is eligible? - All employees
• When Eligible? - At time of annual physical screening, or at discretion of Health Nurse
• Criteria - One year of service or at discretion of Health Nurse

Yearly mammogram

• Who is eligible? - All female employees
• When Eligible? - After one year of service
• Criteria - One year of service or at discretion of Health Nurse

Bone Density Study

• Who is eligible? - All employees
• When Eligible? - After one year of service
• Criteria - 40 years of age and/or at least one established risk factor.
• Contact Person - Occupational Health Services

EKG

• Who is eligible? - All employees
• When Eligible? - After one year of service
• Criteria - None
• Contact Person - Occupational Health Services

Prostate Exam performed by staff physician

• Who is eligible? - All male employees
• When Eligible? - After one year of service
• Criteria - 40 years of age and/or at the discretion of Health Nurse
• Contact Person - Occupational Health Services

Annual Health Assessment:
Hearing Assessment, Blood Pressure Screening, Vision Screening, TB test

• Who is eligible? - All employees
• When Eligible? - Annually
• Criteria - Required of all employees

Vaccination Program:
MMR, Varicella (Chickenpox), TD (Tetanus/Diphtheria), Hepatitis B

• Who is eligible? - All employees
• When Eligible? - At time of employment
• Criteria - Non-immune status to any of the diseases covered by vaccine (except TD which is given every 10 years or as needed).

Flu Vaccine Program

• Who is eligible? - All employees
• When Eligible? - At time of employment
• Criteria - None

Pneumonia Vaccine

• Who is eligible? - Employees meeting criteria
• When Eligible? - At time of employment
• Criteria - Age 50 or above and/or immunosuppressed
