Benefit Summary

 

Full time 72 hours – 80 hours per 2 week pay period

Health Insurance – encompassing medical, prescription drugs & vision
Pre-tax payroll deduction for employee coverage, employee and child(ren), employee and spouse and family coverage.  HMC offers three plan types:  two different PPO plans and the Health Savings Account (HSA).  Insurance benefits include Primary and Specialty Physician office visits, preventative care, diagnostic, hospitalization and prescription drugs.  Insurance is effective 30 days after first day of employment.

Dental Insurance – Dental plan with minimum deductibles include basic, major and partial orthodontia benefits.

Basic Life Insurance – Benefit is based on one times Annual Salary and is provided by HMC at no cost to full time and part time benefited employees.

Supplemental Life Insurance – Available for employee, spouse and children through payroll deduction.

Long-Term Disability – Benefit is based on annual salary and is provided by HMC at no cost to full time employees.

Short Term Disability – Available to full time and part time (benefit eligible) employees through payroll deduction.

Cancer Policy - Available to full time and part time (benefit eligible) employees through payroll deduction.

Long Term Care - Available to full time and part time (benefit eligible) employees through payroll deduction.

Flex Spending Account for Dependant Care and Health Care – Payroll deduction on a pre-tax basis.

HMC Pension and Savings Plan – 403 b (employee contribution).
401 a – Hospital match up to 3% of total compensation.  Eligible for match after 1 year and 1,000 hours worked at age 21.

Paid Time Off  (PTO) – Employees with less than 5 years of service earn 7.38 hours per 2/week pay period (24 days per year).

Extended Illness Bank (EIB) – Eligible employees earn an additional 6 days per year to be used for personal injury or illness, which requires extended absence from work.
