The Admissions Department is located on the first floor of the hospital. The office is open 6 a.m. to 8 p.m. Monday through Friday, and 7 a.m. to 3:30 p.m. Saturday and Sunday. After hours admissions are handled through the Emergency Department Registration area.

To help your admission go smoothly and quickly, registration requires the following: ·

  • Demographic information (i.e. current mailing address and phone number)
  • Insurance Card, Medicare Card, current Medicaid card, or Workers Compensation billing information
  • Written orders from the physician
    Order must include:
    • Test ordered
    • Diagnosis
    • Physicians' Signature
  • Precertification and/or referral number if required by the insurance company

It is the physician's office responsibility to obtain the precertification number. Absence of the required documentation or precertification numbers can cause delays and cancellations. If the physician's office did not give you the written order to bring with you to the hospital, confirm with the physician's office that it was faxed to Henry Medical Center.

Preregistration by phone is attempted on all surgeries and procedures and on some outpatient testing. Information can be faxed by the patient or the doctor's office to 678-604-1055 prior to the date of service.

When you arrive at the Admissions Department you will be required to sign forms consenting to treatment, accepting financial responsibility and assigning health insurance benefits. If the patient is under the age of 18 and unmarried, a parent or legal guardian must be present to sign the documents.

Your physician must be on staff in order to be admitted for surgery and admission to the hospital.
